守門員拇指(gamekeeper’s thumb)或滑雪者拇指(skier’s thumb)是兩種相似情況，都與運動員手部伸展跌倒或滑雪事故時，拇指強烈外展及過度伸展動作，拇指掌指關節尺側副韌帶(ulnar collateral ligament，UCL)斷裂(tear)有關。這一術語源自英國，因為以前獵人(Scottish fowl hunters)要擰斷受傷禽類或小獵物的脖子，引起拇指扭傷及其韌帶鬆弛。

守門員拇指臨床症狀包括拇指掌指關節腫痛(pain)、瘀血(ecchymosis)、關節不穩定(instability of the MCP joint of the thumb)、拇指食指的捏抓無力(weakness of the pinch grasp)(如綁鞋帶或撕紙動作tying shoes and tearing a piece of paper)，關節尺側劇烈壓痛，其嚴重程度與UCL tear程度有關。

1962年，Stener發現拇指尺側副韌帶完全斷裂時，有2/3以上患者的內收拇肌腱膜嵌在斷裂的尺側副韌帶與掌指關節之間(the aponeurosis of the adductor pollicis muscle may be interposed between the bones of the MCP joint and the torn ligament)，引起尺側副韌帶的移位甚至撕脫性骨折(avulsion fracture)；此外，進一步發現拇指旋轉後可導致繼發性關節病。因此，急性拇指尺側副韌帶完全斷裂應手術修復韌帶。

拇指尺側副韌帶的不完全斷裂較完全斷裂常見，治療上只需適度使用拇指人字形石膏，或功能性支架(如 a thumb spica splint or a modified wrist splint)固定(immobilization)4~6星期就可恢復正常。
